GLP-1s and Fatty Liver: Can Ozempic or Wegovy Reduce Liver Fat?

GLP-1 drugs are changing more than body weight. Semaglutide is now an approved treatment for certain people with MASH, and new research is showing what these medications can do to fat stored in the liver.

Yes. GLP-1 medications can reduce fat stored in the liver, and the evidence has become much stronger over the last couple of years.

The biggest development came when Wegovy, the weight-loss version of semaglutide, was approved by the FDA for adults with MASH and moderate-to-advanced liver fibrosis1.

That matters because MASH is not simply “a bit of fat on the liver.” It is a more serious form of metabolic fatty liver disease where fat is accompanied by inflammation and liver damage.

So how did a drug best known for weight loss end up being used for liver disease?

The answer says a lot about how closely your liver and metabolic health are connected. (If you want the broader picture first, here’s how GLP-1 medications affect the liver.)

Why GLP-1s and fatty liver overlap

Fatty liver used to sound like a problem isolated to one organ. We now know that is a poor way to think about it.

Metabolic dysfunction-associated steatotic liver disease, or MASLD, is closely connected with obesity, insulin resistance, type 2 diabetes and abnormal blood lipids.

These are also areas GLP-1 medications can affect.

When someone loses visceral fat, eats less and becomes more insulin-sensitive, the liver’s environment changes too. Less excess energy has to be stored, and less fat can end up accumulating in liver cells.

That is why researchers started looking beyond the number on the scale.

They wanted to know what was actually happening inside the liver.

What happens to liver fat on semaglutide?

The results have become difficult to ignore.

A 2026 meta-analysis looked at 10 semaglutide studies involving 1,908 people with metabolic fatty liver disease. Semaglutide improved several measures, including liver fat, liver stiffness and the enzymes ALT and AST3.

There is also newer data.

At the 2026 European Association for the Study of Diabetes meeting, researchers presented an analysis of people with obesity who had their liver fat measured by MRI.

Among those who started with elevated liver fat, 88.5% of the semaglutide-treated group reached a liver-fat level below 5% after 72 weeks. Average liver fat in the group fell from 8.8% to 3.1%6.

That particular analysis was small, involving 55 people in the MRI subgroup, and it was a post-hoc analysis rather than a dedicated liver trial. Still, it points in the same direction as the broader body of research.

Semaglutide is doing more metabolically than making the scale move.

The bigger story is MASH

This is where things get particularly interesting.

In a major phase 3 trial, 800 people with MASH and moderate or advanced liver fibrosis were assigned to weekly semaglutide or placebo alongside lifestyle changes2.

After 72 weeks, 63% of people receiving semaglutide had resolution of MASH without worsening liver fibrosis.

That compared with 34% in the placebo group.

Improvement in fibrosis without worsening MASH occurred in 37% of the semaglutide group versus 22% of the placebo group.

Those results led the FDA to approve Wegovy for adults with MASH and moderate-to-advanced fibrosis.

This is no longer just a theory about weight loss being “probably good for the liver.” Semaglutide now has direct clinical evidence in a serious metabolic liver condition.

So is Ozempic now a fatty liver treatment?

This is where the brand names can make things confusing.

Ozempic and Wegovy both contain semaglutide.

But they are not interchangeable labels.

Ozempic is approved for specific uses in people with type 2 diabetes and related risk reduction. Wegovy has its own approved indications, which now include certain adults with MASH5.

So somebody reading that “semaglutide helps fatty liver” should not take that to mean that anyone with liver fat should start Ozempic.

Fatty liver also exists on a spectrum. Having some excess liver fat is not the same as having MASH with significant fibrosis.

That distinction belongs with your doctor, not Google.

Is it all because of weight loss?

A lot of the liver benefit probably does come from changing the underlying metabolic problem.

That is not a weakness in the story. It is the story.

In an analysis published in Nature Medicine, weight loss accounted for a substantial portion of semaglutide’s improvement in liver fat and MASH4.

But researchers do not think body weight explains everything. Semaglutide changes glucose regulation, insulin sensitivity, appetite and lipid metabolism, and researchers continue to study whether some liver effects go beyond weight loss alone.

What is becoming clearer is that the liver responds when metabolic health changes.

That is important even if you have never been diagnosed with fatty liver.

Your liver sits directly in the middle of how your body handles fat, sugar and energy — it’s worth knowing what your liver actually does. GLP-1 research is making that connection much harder to overlook.

Where does liver support fit if you’re already on a GLP-1?

This is where we think the conversation gets more useful.

A GLP-1 medication and a liver supplement are not trying to do the same thing.

Semaglutide can produce major metabolic changes, including weight loss and reductions in liver fat. Olsom isn’t designed to replicate that.

We built Olsom for a much simpler job: consistent daily liver support.

Olsom Liver combines three ingredients we deliberately chose from the research:

NAC · 600 mg

A precursor used by the body to produce glutathione.

Milk Thistle · 265 mg

One of the most researched botanical ingredients in the liver category.

Artichoke Leaf · 500 mg

Studied for its role in bile flow, fat digestion and liver-related markers.

Two capsules a day. No cleanse cycle. No giant ingredient list added for show. (Here’s why we skip liver detoxes and cleanses.)

If you’re taking a GLP-1, your medication should be managed with your clinician. Olsom sits alongside the bigger things you’re already doing rather than trying to replace them.

For us, that is what liver support should look like.

What should you take away from all of this?

The interesting part of the GLP-1 story is no longer just weight loss.

We’re seeing what happens when metabolic health improves across the body, and the liver appears to be one of the organs that can benefit substantially.

For some people with MASH, semaglutide has now gone even further and become an actual treatment option.

For everyone else, the research is a useful reminder of something we talk about a lot at Olsom:

Your liver isn’t separate from your metabolic health. It’s right in the middle of it.

Frequently asked questions

Do GLP-1 medications help fatty liver?

Research shows that GLP-1 medications, particularly semaglutide, can reduce liver fat. Semaglutide has also shown benefits in people with MASH, a more serious form of metabolic fatty liver disease.

Does Ozempic reduce liver fat?

Ozempic contains semaglutide, and research on semaglutide has shown reductions in liver fat. However, Ozempic itself is not the semaglutide product FDA-approved for MASH. Wegovy has that indication for certain adults.

Is Wegovy approved for fatty liver disease?

Wegovy is FDA-approved for adults with noncirrhotic MASH and moderate-to-advanced liver fibrosis. That does not mean everyone with simple fatty liver needs Wegovy.

Can GLP-1 medications lower ALT and AST?

Studies and meta-analyses have reported reductions in liver enzymes including ALT and AST with semaglutide. Liver enzymes can change for many reasons, so abnormal results should be interpreted by a healthcare professional.

Does losing weight reduce liver fat?

Weight loss can substantially reduce liver fat and is an important part of managing metabolic fatty liver disease. Research suggests weight loss accounts for a large portion of semaglutide's liver benefit.

Should I take a liver supplement while using a GLP-1?

There is no general requirement to take a liver supplement because you use a GLP-1. Olsom is designed for people who want daily liver support alongside their broader health routine, not as a replacement for GLP-1 medication or medical care. Discuss supplements with your clinician if you take prescription medication.
